Номер дома с дробью может означать не только номер/строение, но и просто номер дома (в старых переулках Москвы множество домов снесено, а остальные имеют номера вроде 7/15, чтобы нумерация не сбивалась). Произносить такой номер надо "Building twenty five forward slash two, ulitsa Yeremenko". 

Про письменную форму уже высказались предыдущие ораторы :) - если адрес пишется латиницей для них, то принимаем их порядок (от имени до страны), если адрес пишется для наших почтовых работников, используем наш порядок расположения адресных данных.
